 1.         Payments

I would like to remind parents of the agreed dates for payments (£80 instalments).

Deposit – Nov 2009 (£80)

Followed by 4 payments of £70 - January, February, March, April

(By middle of each month)

The final payment is therefore due by the middle of April.

2.         Passport Photographs

All pupils under the age of 16 will travel on a group passport.

To enable me to proceed with a group passport application, I now require 2 colour passport sized photographs of your child.

AS PREVIOUSLY STATED A PAYMENT OF £3 PASSPORT/ADMINISTRATION COSTS WILL BE COLLECTED AT THE FORTHCOMING PARENTS’ EVENING EARLY MAY (date to be confirmed).

NB    on the back of one photo please print the name of your child AND town of residence.

            A pro forma must also be completed for each child under 16. (See attached sheet).

It is vital that photographs and pro forma are returned by the end of the week beginning Monday 1st March at the latest.

3.         E111

New regulations state that E111’s must be in the new format and we are therefore required to carry a EUROPEAN HEALTH INSURANCE CARD for each individual.  Please check if there is an expiry date.

Please refer to attached sheet for options on how to apply. We recommend that   you do this as soon as possible as we would require these cards by the end of April.

From experience an application made on-line is much quicker-although we would remind parents that the busy season may be just commencing.

Application For European Health Insurance Card (EHIC)

If you are a UK resident, you are entitled to medical treatment that becomes necessary, at reduced cost or sometimes free, when temporarily visiting a European Union (EU) country, Iceland, Liechtenstein, Norway or Switzerland. Only treatment provided under the state scheme is covered. However, to obtain treatment you will need to take a European Health Insurance Card (EHIC) with you. Please note: Not all UK residents are covered in Denmark, Iceland, Liechtenstein, Norway or Switzerland. The quickest way to receive your EHIC, free of charge, is to apply on  website by clicking on the 'Apply Now' link below.

Apply Now for your EHIC or to renew your expired card.

In order to apply ensure you have the NHS or National Insurance Number, Surname/Family Name, Forenames and Date of Birth of each applicant.

Alternatively, you can apply by calling 0845 606 2030.If you are a non-EEA national you can only apply by post as you need to provide further documentation that supports your application. You can pick up an application form from the Post Office.

Only one application by either post / internet or telephone is necessary. If you have made any prior applications and are yet to receive a card, please call our enquiry line on 0845 605 0707 for your EHIC or to renew your expired card.

If your card is stolen or lost please contact our Enquiries Line on 0845 605 0707 or if calling from abroad +44 191 212 7500.

 

 

4.         Emergency Contact /Medical conditions information will be required end of March.

            This information will be issued separately.

This will be updated again prior to our departure.

I propose to hold a Parents’ Information Evening in May.

A detailed Information Booklet will be issued at that meeting.

Spending money of £120 should be paid at the meeting, along with £3 passport/Admin costs and not paid to M-Line.

A prompt return of photos/passport pro formas and E111’s would greatly appreciated to enable us to deal with paperwork timeously.

Passport info should be returned no later than week beginning Monday 1st March.

EHIC Health Cards- as soon as possible especially if not required for the Easter Holidays.

Please inform us if this is the case.

I appreciate that a lot of information/form filling is required at this time, but I am sure you will appreciate the reasons for this and thank you in advance for your cooperation.
